As Oak Street Health’s real estate partner, Avenue utilized the strategic expansion model it created for the primary care provider to further grow its Indiana presence in Fort Wayne. In partnership with Humana, this center features vast community and educational space, clinical area, and a Humana Market Point office. Avenue provided site selection, turn-key development services including a historical building renovation, and FF&E procurement for this lease development.

Oak Street Health

Oak Street Health was founded in 2013 to be an industry-leading primary care platform built for a value-based healthcare ecosystem. Oak Street operates a network of centers in predominately urban markets without many healthcare resources, and focuses on treating adults on Medicare for preventative care, mental health and well-being. Today, Oak Street Health operates over 50 locations and growing across eight states.

Revitalizing Downtown

Avenue acquired and renovated a building in disrepair which was originally constructed in multiple sections dating back to the early 1900’s. The historical building was transformed to its initial grander, and spurred economic development in the subsection of downtown Fort Wayne. The center opened in the Fall of 2016 in time for the Medicare open enrollment period.

Location

The facility is located at a highly visible and well-traveled stop-lit intersection of Washington & Clay. Washington is one of the most-traveled, one-way, thoroughfares into downtown Fort Wayne. The facility’s location allows for easy walking access from downtown neighborhoods as well as nearby social service centers and hospitals.

The Back-story

In May 2016, Avenue purchased the facility from a local football legend, and revitalized the prominent City corner to it’s historical roots. The property is leased to a  subsidiary of Humana, and operated by Oak Street Health and Humana’s Market Point office.
