113-BED (50-BED CON) SENIOR HOUSING & SKILLED NURSING FACILITY
Avenue specializes in procuring certificate-of-need (CON) licenses and other entitlements in high barrier to entry markets. Trilogy and Avenue collaborated to expand the respected operator in their growing central Ohio presence, by sourcing fifty off-market CON beds in Franklin County to allow for a skilled nursing component to the new development. Avenue directed the regulatory process for transferring the bed licenses with the state, and provided turn-key development, financing and construction activities for the new 113-bed senior housing community. Now named Smith’s Mill Health Campus, Trilogy continues to provide family centered living opportunities across the care continuum including independent living, assisted living, memory care and post-acute rehabilitation.
Smith’s Mill Health Campus is strategically located in New Albany’s healthcare corridor across from a specialty surgical hospital, Mount Carmel New Albany, allowing for cohesive progression for patients needing extended care or rehabilitation between the acute care and post-acute settings. This turn-key development in the Trilogy portfolio opened to residents in December 2019.
