Gary’s mission is to transform healthcare via disruptive business models in housing, wellness delivery, local food production and other innovative methods. As Executive Advisor, Gary consults with Avenue Development in its clients in their efforts to develop new skilled nursing, senior housing, active adult and intergenerational communities across the country. He also works with operators of these properties and health systems with integrated post-acute care to improve the delivery and coordination of care for our elders through program implementation and operational budget assessments.
Gary strongly believes the combination of sustainable local food production, preventative healthcare delivery via the direct primary care model, and housing innovations can result in the development of communities that improve people’s wellness and longevity. He has combined these passions in his efforts as the Managing Director of Organic Health Investments and Owner of Rolling Fork Organic Farm, and coordinates numerous programs with various organizations and health systems to address the social determinants of health; access to local/healthy food, improving the quality of housing and improving socialization in local communities.
A constant innovator in senior housing, Gary has worked with Dr. Bill Thomas, founder of the Green House Project, which has now led to becoming an investor and advisor to Minka Homes which will revolutionize affordable housing for all ages. He is the former Senior Vice President of Mainstreet Health, and prior to joining Mainstreet provided consulting services to acute care hospitals and post-acute providers to address hospital re-admissions, strategic business development and new revenue cycle opportunities via GSS Management Group. Gary has an esteemed operations background, heading operations for Encore Healthcare, with 30 skilled nursing centers and 5 long term care hospitals and was the co-founder and Executive Vice President of Administration for American Care Communities, an assisted living start-up, growing from one AL/IL property in North Carolina to 28 across 4 states and selling the company to a public senior housing firm in 1999. Gary began his career with Beverly Enterprises, now Golden Living, involved with acquisitions, development and construction over a 10-year period. During this time, Beverly grew from 220 facilities to over 1,000 senior housing properties in all states except New York and Alaska.
Mr. Smith earned a Bachelor of Political Science degree from the University of Florida.
